The following information outlines the process for submitting the paper based applications

• If unable to complete the online application, download and enter your information into the PDF application form fields
  
• After completing the form, print the completed application form
  
• Print the Application Form and all of the additional required forms for the membership type you are requesting.

• For 1st Senior applications:  Include the completed application and all additional required forms
  

• $25 payment for the background check
• Completed 1st Senior Application Form            
• 1st Senior Membership waiver form w/ applicant signature
• Two (2) Fully completed / signed Current 1st Senior Member Reference Forms

• For 2nd Senior applications:  Include the completed application and all additional required forms
  

• $25 payment for the background check
• Completed 2nd Senior Application Form
  
• 2nd Senior Membership waiver form w/ applicant signature

• For Junior applications :  Include the completed application and all additional required forms 

• Completed Junior Application Form
• Junior Waiver form - signed by the sponsoring 1st Senior
